After the appointment of your doctor, the office of your doctor subjects an invoice (also called a complaint) to your insurance company. A complaint enumerates the services your doctor provided to you. The insurance company employs information in the complaint to pay the doctor these services.
When the insurance company pays your doctor, it could send you a report/ratio called an explanation of the advantages, or to EOB, which shows you what it. You must be able to read and include/understand the EOB to know what your insurance company pays, which it does not pay, and why. A EOB is not an invoice.
